Acute Risk Management Clinic for Seniors

(Pilot Project)

The Geriatric Program Council of Capital Health recognizes that there are gaps in services for seniors who have complex, acute health needs.

Our goal is to provide timely and appropriate services for seniors whose independence is threatened by acute illness.

  • through referral from other health service agencies

Who are the Clinic team members?

Health care professionals experienced in geriatrics including:

  • Registered Nurses

  • Licensed Practical Nurses

  • Social Workers

  • Rehabilitation Therapists

  • Pastoral Care/Chaplains

  • Clinic-based family physicians

  • Patient's family practitioner

  • Seniors' Volunteer Advisors

  • Laboratory & Radiology Services

  • Geriatricians and other medical specialists

What services are provided?

  • geriatric consultation

  • case planning in collaboration with family doctor and community-based services

  • linkage with appropriate community programs and services

  • short-term health care management to bridge waiting lists or cover gaps in services

  • professional education and research opportunities

  • interdisciplinary resources to support both patient and family physician

What results does the Clinic expect?

  • fewer admissions to hospital from emergency

  • ical services for family practitioners and patients under their care

Clinic hours

Clinic location

  • the pilot project will initially provide services from 9:00 a.m. - 5:00 p.m., 7 days a week

  • patients will be seen at their convenience and when necessary, within 24 hours of referral

Did you know?
